Applications will not be considered complete and will not be accepted until all the following information is received:

1. Completed Application
2. SSI documentation for any disabled members of the household
3. Signed Authorization for Release of Information signed by all members of household (18+)
4. Signed Resident Screening Verification
5. Verification of Income
6. Social Security Cards (copy)
7. Enrollment Cards (copy)
8. Rolette County Background Check (18+)
9. Turtle Mountain Tribal Court Background Check (18+)
10. Updates required every (6) six months to remain on the waiting list
